• Full two-year address history (i.e. Full address, rent or own, how long, current payment)
• If renting from apartment complex or management company, landlords name, address and phone number
• If renting from individual, landlords name, address and phone number along with 12 month cancelled checks, both sides

Income:
• Full two-year work history (i.e. Company name, address, phone number, position, dates worked and income)
• 30 days most recent pay stubs with year-to-date earnings
• Most recent 2 years W2’s from all jobs
• YTD P & L (if self-employed)
• 2 years complete tax returns (if self-employed)
• Copy of business license (if self-employed)
• CPA letter with CPA license number (if self-employed)
• Copies of Social Security, retirement or VA benefits awards letter (if applicable)

Assets:
• Most recent 2 months bank statements, all pages
• Most recent 401K, IRA or other retirement statements
• Most recent stocks/bonds/investment statements

Miscellaneous items, if applicable:
• Divorce decree
• Child support court order
• Complete bankruptcy papers
• Proof collections/judgments paid in full or in repayment
